You pay an annual fee and gain access to great dental care at significantly reduced rates on services. In many cases, it is more affordable than dental insurance. Coverage

Preventative Services – 100% Covered through Membership Fee

• Oral Exams – 2 per calendar year
• Prophylaxis (Cleanings) – 2 per calendar year
• X-Rays – 1 set per year
• Flouride Treatment – 1 per calendar year
• Oral Cancer Screening

Basic Services: 20% discount off regular service fees

• Fillings
• Sealants
• Repairs on Crowns and Dentures

Major Services: 25% discount off regular service fees

• Root canals and other surgeries
• Crowns and Bridges
• Dentures
• Cosmetic Dentistry

Other Services

• Whitening – Professional in-house whitening for only $249 (normally $350)
• $75 Credit to your account for referring others to the program

Membership Fees

• Individual: $450 per year or only $37.50 per month*
• Individual + 1: $850 per year or only $71 per month*
• Each Additional Family Member: $275 per year or an additional $23.00 per month*

*Fees for the year are due at sign-up of the program. Membership is good for 1 year from enrollment date.

The PFD Dental Program is NOT insurance. The plan in not considered to be a qualified health plan under the Affordable Care Act. Since there is no paperwork or reimbursement, you must pay for the service at the time it’s provided. You will receive the discount off the usual and customary fees when you pay.
